This role is critical in maintaining continuity of care and supporting the administrative and operational needs of the care management process.<br><br><strong>Key Responsibilities<br><br></strong><ul><li>Conduct monthly outreach calls to assigned members to monitor well-being and document updates.</li><li>Assist Care Managers in completing and submitting service authorization requests.</li><li>Follow up with Primary Care Provider (PCP) offices to obtain required documentation for service requests.</li><li>Coordinate with service providers to confirm delivery and implementation of authorized services.</li><li>Support members with Medicaid recertification processes to ensure uninterrupted enrollment and benefits.</li><li>Assist in scheduling and coordinating home visit assessments as needed.</li><li>Compile and share relevant member information and reports with other departments to support interdisciplinary collaboration.</li><li>Maintain accurate and timely documentation in care management systems.<br><br></li></ul><strong>Required Qualifications<br><br></strong><ul><li>2–3 years of experience in healthcare coordination, case management support, or administrative roles in a clinical or social services setting</li><li>Bilingual- Mandarin and/or Cantonese - May be required to take and pass a language assessment - verbal and written.<br><br></li></ul><strong>Preferred Qualifications<br><br></strong><ul><li>Strong communication and interpersonal skills, especially with vulnerable populations.</li><li>Ability to manage multiple tasks and follow through on assignments independently.</li><li>Familiarity with Medicaid processes and service authorization workflows.</li><li>Proficiency in using electronic health records (EHR) and care management platforms.</li><li>Detail-oriented with strong organizational and documentation skills.</li><li>Ability to work collaboratively with clinical and administrative teams.</li><li>Computer literacy to navigate through Internal/external computer systems, including Excel and Microsoft Word<br><br></li></ul><strong>Education<br><br></strong>High School Diploma or equivalent experience<br><br><strong>Anticipated Weekly Hours<br><br></strong>40<br><br><strong>Time Type<br><br></strong>Full time<br><br><strong>Pay Range<br><br></strong><strong>The Typical Pay Range For This Role Is<br><br></strong>$18.50 - $38.82<br><br>This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
