Key Responsibilities Conduct comprehensive patient assessments via telehealth platforms to identify needs and care goals. Develop, implement, and monitor individualized case management plans in collaboration with patients, families, and healthcare teams. Coordinate patient care across various settings, including hospitals, rehabilitation facilities, home health, and physician offices. Advocate for patients' needs and ensure they receive appropriate and timely healthcare services. Educate patients and families about their conditions, treatment options, and available resources. Facilitate communication between patients, providers, payers, and community resources. Monitor patient progress and outcomes, adjusting care plans as necessary. Adhere to all relevant healthcare regulations, including HIPAA. Requirements Associate's Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred. Current and unrestricted Registered Nurse (RN) license in the United States.
