Together with our patients, physicians, partners, and communities, we are creating a more just, equitable, and innovative healthcare delivery system.</p> <h2>Job Summary and Responsibilities</h2> <p><strong>This position is remote with 50%+ travel. </strong></p> <p><strong>Incumbents must reside in the Washington market</strong></p> <p> </p> <p><strong>Region Director, Care Coordination</strong></p> <p>As the Region Director, Care Coordination, you will be a strategic leader responsible for <strong>optimizing patient-centered care across all hospitals in your assigned region.</strong> You will ensure high-quality care, operational excellence, and regulatory compliance within Care Management functions.</p> <p><strong>Key responsibilities include:</strong></p> <ul> <li><strong>Strategic Leadership:</strong> Develop and implement comprehensive plans to drive patient throughput, efficient discharge planning, and readmission prevention.</li> <li><strong>Operational Oversight:</strong> Provide clinical direction for patient flow and ensure consistent application of systemwide standards and best practices for care coordination.</li> <li><strong>Regional Expertise:</strong> Leverage deep understanding of local regulations and resources, building relationships with state agencies and community partners.</li> <li><strong>Collaboration & Partnerships:</strong> Work closely with hospital and system leaders, as well as external stakeholders (payers, community resources), to achieve organizational objectives and reduce denials.</li> <li><strong>Performance Improvement:</strong> Drive continuous improvement in care management metrics, utilizing data analytics to enhance operations and patient outcomes.</li> <li><strong>Team Development:</strong> Foster a culture of excellence, provide staff development, and ensure appropriate staffing models.</li> <li><strong>Compliance & Ethics:</strong> Guarantee adherence to all regulatory requirements and uphold the organization's mission and values.</li> </ul> <p>You will be expected to demonstrate exceptional executive leadership, foster interdisciplinary collaboration, and champion relationships to deliver critical results.</p> <h2>Job Requirements</h2> <p><strong>Required Education and Experience</strong></p> <ul> <li>Masters Other Master's degree or equivalent education and experience in nursing or healthcare/business related field. </li> <li>Bachelors Of Nursing and a current RN license</li> <li>10 years in acute care management including 5 years in a leadership role overseeing multiple facilities or a regional structure.</li> <li>Accredited Case Manager Certified (ACM)</li> <li>Certified Nephrology Nurse (CNN)</li> <li>Proven success in developing and implementing large-scale care management strategies.</li> </ul> <p> </p>
